Congratulations on your Pregnancy
After informing your GP of your pregnancy you will need to self refer to the local Midwifery Service.
Please see instructions below which have been prepared by our local Midwifery Service - the Amethyst Midwifery Team.
Please follow this link to complete the Midwifery Service Self Referral Form online.
Once you have completed the form, the Midwifery Team will be in touch within 5-7 working days to arrange a booking appointment for when you are between 8 and 10 weeks pregnant. An ultrasound scan will be offered to you at around 12 weeks to ascertain when your baby is due.
If you are unable to complete the form or have any difficulties answering the questions, please call the Bluebell Birth Centre, at Warwick Hospital, and we will assist you:

We look forward to meeting you. Thank you from the Amethyst Midwifery Team.
Keeping Well in Pregnancy
Folic Acid
In the meantime, it’s recommended that you take folic acid tablets (400 micrograms) until your first scan at around 12 weeks. This is widely available from pharmacies and supermarkets. If you want to get your folic acid from a multivitamin, make sure that it is suitable to take during pregnancy and the tablet doesn’t contain vitamin A (retinol).
Please see your GP to be prescribed a higher dose of folic acid, if:
You have diabetes
You have a higher BMI
You are taking anti-epilepsy medication
You have sickle-cell anaemia, thalassaemia or thalassaemia trait
You have a neural tube defect
The baby’s biological father has a neural tube defect
A previous pregnancy has been affected by a neural tube defect
You or the baby’s biological father have a family history of neural tube defect
Vitamin D
The Department of Health also advises you to consider taking a vitamin D supplement. In general this should be 10 micrograms (400 units) a day.
Please consult with your GP as you may require a higher dosage, if:
You have increased skin pigmentation
You have reduced exposure to sunlight
You have a higher BMI
You have an increased risk of pre-eclampsia
