For Children aged 5-16
Referrals for NHS Autism and ADHD assessments in children should be completed by the child’s school. The surgery DO NOT make these referrals so we wont be able to help with this.
More recently we have been asked to make referrals for Autism and ADHD assessments to private providers via the ‘Right to Choose’ scheme. Some parents have chosen this route as a quicker means of undertaking an assessment.
If you wish to request a referral for an Autism and/or ADHD assessment with a private provider via the ‘Right to Choose’ scheme:
Please download and complete the Neurodevelopmental School Age Service Referral Form.
Please identify your right to choose provider. We cannot help with this so you will need to find your own provider. Please make sure the provider is able to assess your child based on their age. Of note you may wish to consider providers who are CQC registered only, as we do not support medication requests at a later date from providers who are not CQC registered.
Please email your completed referral form and details of your ‘Right to Choose’ provider to the surgery.
For Those Aged 16-25
We are able to make referrals for individuals aged 16-25 for an Autism and/or ADHD assessment. This can be done via the NHS or via the ‘Right to Choose’ scheme. Please note that our local NHS will only allow ‘Right to Choose’ referrals to organisations with a face-to-face assessment facility in Coventry & Warwickshire, so please check with the provider first, before requesting a referral.

For Those Aged 25+
We are only able to refer for an Autism assessment at this age. This can be done via the NHS or via the ‘Right to Choose’ scheme.
