Disability, Reasonable adjustments and communication needs

We want everyone to be able to use our services safely, effectively and with dignity.

If you are disabled, have an impairment or sensory loss, a learning disability or difficulty, or another condition that affects how you access healthcare, please tell us what support would help you.

What is a reasonable adjustment?

A reasonable adjustment is a change we can make to reduce or remove a barrier you experience when using our services.

Everyone’s needs are different. We will discuss what works for you and make adjustments wherever practicable. Some adjustments, such as longer appointments, may depend on your clinical needs.

Reasonable adjustments could include:

  • allowing more time for an appointment where clinically appropriate

  • contacting you in your preferred way

  • providing letters or information in large print, Easy Read, email or another accessible format

  • arranging Braille through an external provider

  • arranging a British Sign Language interpreter or language interpreter

  • helping you complete NHS forms

  • offering a quieter place to wait, where available

  • helping you enter or move around the building

  • collecting you from the waiting area or escorting you to a consultation room

  • considering appointment times that work with your carer or support person

  • ensuring there is suitable space for an assistance dog

  • discussing alternative arrangements if your disability makes it difficult to attend the surgery

These are only examples. Please tell us about any other adjustment that would help you.

Accessibility at the practice

Our facilities and support include:

  • an induction loop at reception

  • clearly marked, wider disabled parking bays

  • chairs with arms to help people sit down and stand up

  • accessible consultation arrangements

  • guide and other assistance dogs being welcomed

  • a wheelchair available at the main surgery and at Southam Clinic

  • assistance from staff if you need help accessing the building

If you cannot use the online form, please contact reception. A carer, family member or representative can also help you tell us what you need.

What happens to your information?

We will record your reasonable adjustments and communication needs on your clinical record so that relevant practice staff can see them and provide the right support.

Where appropriate, relevant information may also be shared with NHS or publicly funded social care services involved in your care, in line with NHS information-sharing rules. This can help other services prepare for your needs and reduce how often you have to explain them.

You can tell us if you object to your reasonable adjustment information being shared. We will handle your information confidentially and in accordance with NHS data protection requirements.

If your needs change

You can have more than one reasonable adjustment. Your needs may also change over time.

Please contact us whenever you need to add, change or remove an adjustment. We will review your needs and keep your record up to date.
